What causes PCOD and PCOS?

In simple words, let's imagine hormones as the orchestra conductors of the body. They keep every process in rhythm: your metabolism, mood, menstrual cycle etc. When the hormonal signals lose its rhythm, especially between brain and ovaries, conditions like PCOS & PCOD can appear. Let's understand the normal Hormonal physiology of Menstruation & Ovulation.

Your Menstrual cycle is carefully choreographed dance by the hormones from the brain and ovaries, called Hypothalamic-Pituitary-Ovarian (HPO) Axis. The Hypothalamus releases GnRH (Gonadotropin Releasing Hormone) which tells the Pituitary gland to release FSH (Follicle Stimulating Hormone)  & LH(Luteinizing Hormone). In the Follicular Phase(Day 1-14), FSH helps the follicles(Which has egg inside it) in your ovaries to grow. On maturing these follicles produce Estrogen, which thickens the Uterine lining. On the 14th day, the Estrogen reaches the peak and triggers sudden LH surge, causing one mature egg to release - This is Ovulation.

During the Luteal Phase(Day 15 - 28), the empty follicle turns into Corpus luteum, which produces Progesterone, which calms the body & prepares the Uterus for pregnancy. If Pregnancy doesn't happen, Progesterone and Estrogen drop, leading to the next period - and the cycle begins again. 

What actually happens in PCOD/PCOS?

In PCOD/PCOS the normal Rhythm of HPO Axis is disrupted. The hypothalamus sends too frequent GnRH pulses, which in turn cause release of more LH & Less FSH. High LH stimulates the Theca cells in the ovaries to produce Androgens (Testosterone). When the FSH is low, the follicles in the ovary don't mature and the ovulation doesn't happen. Hence many immature follicles accumulate in the ovaries, giving it "Polycystic" appearance in scan reports. As there is no ovulation, there will be menstrual irregularities and increased androgen gives rise to acne, oily skin, hair fall, weight gain and hirsutism.

Difference between PCOS & PCOD in simple terms:

IN PCOD, your ovary is like a fruit orchard. Every month they try to grow many eggs, but most of them remain unripe and small. These tiny, unripe eggs turn into cysts. In normal menstrual cycle, one or two follicles in the ovary mature to release one egg during ovulation. In PCOD, many follicles begin to mature but none of them reach full maturity. These immature follicles stays in the ovaries as small fluid-filled sacs called cysts. There may be menstrual irregularities & weight gain, but the overall metabolic function often remains normal.  

In PCOS, your ovary is like a same fruit orchard, but this time the weather itself has changed. The soil(metabolism), sunlight(hormones) and even the gardener(Brain & ovey connection) are affected. In PCOS, it's not just the ovaries are affected, your metabolism, Insulin response and the whole hormonal system is affected.
